Certificate, in Polish, from 8 July 1941, given to a Jewish dentist whose house and clinic were at 5 Carmelizka St., Warsaw, which was at that time under Nazi occupation.
The certificate details the names of his parents and grandparents, his city of birth and additional details. The dentist’s picture is attached to the top of the document.
